Summary: We're trading PDF formatting for HTML formating

Considering were we are in the cycle, I'll take this compromise,
however we need to follow up with a bug to address this issue.

The whole point of docbook is to make the source independent
of the target, it's the XSLT templates that are supposed to
handle this transformation.

The concepts here appear directly applicable to our work, I
was unable to get them working. Worth further investigation.

Regarding the warnings:
Page 51: Unresolved ID reference "priority-checker-conversion-table" found.
Page 76: Unresolved ID reference "useful-multipath-command-options" found.

These ID's are well defined, and work fine under the HTML, the links are
declared forward of the target, which shouldn't be a problem, and is also
used throughout the document without issue. One thing I noticed in the
rendered PDF.

For whatever reason, the template engine can't figure out what page we're on.
This might be the reason for the warning. We can live with it.

I took dsmythies branch and normalized all the col widths so the tables have
consistent formatting, again, this is at the expense of HTML formatting,
which is now sized dynamically to fit the text.

Also fixed a formatting error where prio_arg was cited as a link, it's
supposed to be wrapped in a parameter tag, without it, the text was
rendered differently and stood out in the itemized list. Fixed now.
