Have get_all_doc_ids return the current db_generation just like whats_changed.

So now the only real difference between the two is that get_all_doc_ids doesn't return
the ids for deleted documents.

Remove the ProtocolEncoderV1.encode_request method, as this is
being logically done by the responder and the client classes.
Although both of them might want to share functionality, since
the data is almost the same on both sides.
Remove a superfluous test.
