Add stubs for the remaining protocols that we intend to support.

No tests yet because these stubs don't even really have anything to test.
They are just here so that barry's Facebook account doesn't throw exceptions
when he tries to run the test suite ;-)

Merge lp:~robru/gwibber/avatars to support downloading an avatar image from
file, with conversion to PNG and caching.

Changes by barry:

* Clean up unused import.
* Instrument urlopen() mock with a call count for easier testing.
* Ensure that TestAvatar uses a temporary cache directory so as not to be
  polluted by the user's environment.
* Add separate tests for cache hits and misses.
* Style nits.
