gettextize: do not associate the strings by their order in the POs, but by
their order in the documents. This permits to gettextize documents which have
the same structure, but where a string have multiple translations, or when
different strings have the same translation.
If multiple translations appear for a string, both strings are proposed in

Do not put the header at the top of the file, but just before the .TH
macro. This will preserve the indications on how to process the page if
the first line contains something like: '\" t

Even if this warning is not at the top of the file, I think it is still
visible for any translator who would like to translate the generated man

Fix a typo (did not compile; remove a 'my')
Remove <quote> from the list of placeholder tags. The robustness was
tested on the debian-installer: after a po4a-normalization, the .txt and
.html outputs of the d-i manual were equivalent.

Implement placeholders: this permits to specify that footnotes must not be
translated inside a paragraph, but are translated separately and a
<placeholder0> tag will be put where the footnote must be installed.

This is still beta. The user can't specify what tags must be replaced by
<placeholder> tags: currently only <footnote>, and <quote> (the later for
testing purpose, so that I can try placeholders inside other
placeholders).
It's probably not very robust and need more testing.

Move the punctuation out of the E<...> markups. This should be easier for
the translators.
